ATSB: Qantas A380 Flew Two International Sectors With a Work Light Left in Its Wing

SYDNEY — Three separate safety checks failed to catch a maintenance work light left inside the wing of a Qantas A380. The aircraft flew two international sectors before the light was found. The Australian Transport Safety Bureau (ATSB) detailed the failure in a final report released August 17. It marks the conclusion of an investigation into Qantas A380 tool control.

The Makita LED work light was roughly 33 centimeters tall. It was left in the left wing of Qantas A380-842 VH-OQK. That happened after maintenance on the aircraft’s air conditioning system at Sydney Airport on January 7, 2026. The aircraft then flew to Dallas/Fort Worth and back, a round trip of nearly 27,000 kilometers. The light was found and removed on January 9. No damage or injuries resulted.

Three Checks, Zero Catches

ATSB investigators traced the miss to a chain of missed opportunities rather than a single mistake. Maintainers completed a foreign object clearance inspection at the end of the air conditioning work. They didn’t spot the light. It had been switched off and left in a hot, cramped wing cavity.

The gap wasn’t caught when tools were returned to the crib. It also wasn’t caught during the end-of-shift unreturned-tool check. That check exists specifically to backstop a missed inspection.

“Even well-established processes can be vulnerable when they rely solely on consistent human performance,” ATSB Chief Commissioner Angus Mitchell said. Maintenance tasks are “often conducted in challenging environments and under operational pressures,” he said. “Tool control systems and procedures must account for this variability,” Mitchell said. They need “safeguards to detect and recover from errors.”

A Software Gap Behind the Signature

The report’s most pointed finding involves what happened next. A licensed aircraft maintenance engineer issued the certificate of release to service (CRS). That’s the sign-off that clears an aircraft to fly. The work light remained unaccounted for.

Qantas’s maintenance system had no automated alert for the certifying engineer. It couldn’t flag that a tool was still checked out. It also couldn’t flag that an unreturned-tool report existed before the CRS was signed. ATSB named this gap a formal safety issue, AO-2026-004-SI-01. The sign-off relied on the engineer pulling up that report by hand. That step didn’t happen. Engine work and departure prep got in the way.

Not the First Time

This is the second time in three years the ATSB has investigated a Qantas A380 for a maintenance tool left inside the aircraft. FODNews reported in March when the ATSB opened this investigation. Separately, a December 2023 incident saw a 1.25-meter nylon compressor turning tool left inside the engine of a different Qantas A380. That tool flew 34 flight cycles before discovery. Tool control failures aren’t unique to Qantas, either. A missing downlock safety pin contributed to a Lufthansa 787-9’s nose gear collapse at Frankfurt in June.

Qantas’s Response

Qantas voluntarily reported the January incident. The airline issued a safety directive in February changing its tooling control procedures. It introduced a mandatory pre-clearance check confirming all tooling is accounted for before an aircraft is released to service. Qantas also formed a working group evaluating digital tool-tracking technology.
